I, Jacob E. Rabinowitz, hereby attest under penalty of perjury that I am at least partially handicapped under the definition promulgated in 42 U.S.C. § 12102(1) and 28 C.F.R. § 36.105(1). I am not required by the laws of this state or by the laws of the United States to disclose the nature of my disability in order to receive services from any public agency, public accommodation, or employer. Under the definition promulgated in 42 U.S.C. § 12131(1)(a), the City of Los Santos is a public entity that is prohibited under 28 C.F.R. 35.130(b)(8) from discriminating on the basis of disability status by creating eligibility criteria subject to evaluation on the basis of specific disabilities.
